PURE AQUAOX CONSIGNMENT AGREEMENT
This Consignment Agreement (“Agreement”) is entered into as of the date signed below between Pure Aquaox, LLC (“Consignor”) and the undersigned organization or business (“Consignee”).
1. Purpose
The Consignor agrees to provide TreeVive™ and/or other Pure Aquaox products (“Products”) to the Consignee on a consignment basis for the purpose of retail sale or fundraising.
2. Delivery and Ownership
Products remain the property of Consignor until sold by the Consignee.
Consignee agrees to maintain accurate inventory records and to notify Consignor weekly of items sold or remaining in stock.
3. Pricing and Profit Share
Suggested retail price: $20.00 per bottle
Consignee retains $8.00 per bottle sold as their fundraising profit.
Remaining $12.00 per bottle is payable to Consignor.
4. Payment, Reporting & Authorization
Consignee agrees to submit a sales report to Pure Aquaox every Friday summarizing all Products sold during the prior week. Based on this report, Pure Aquaox will process payment for the balance owed on all reported sales.
Consignee hereby authorizes Pure Aquaox, LLC to automatically charge the credit card provided on file each Friday for the total amount due from the weekly sales report. This authorization shall remain in effect until all consigned inventory has been sold and payment has been made in full, or until Consignee provides written notice of cancellation at least seven (7) days prior to the next scheduled payment.
Consignee agrees that all payments are due and payable upon reporting, and that failure to submit weekly reports may result in account suspension or collection of the full value of consigned inventory.
Unsold inventory may be returned in original, saleable condition upon request.
5. Term
This Agreement shall remain in effect until all Products are sold or returned, unless terminated earlier by either party with written notice.
6. Product Care and Representation
Consignee agrees to store and display Products in a clean, temperature-controlled environment and to represent the brand in a professional and ethical manner.
7. Liability and Risk
Consignee assumes responsibility for loss or damage to Products after delivery. Pure Aquaox is not liable for indirect, incidental, or consequential damages related to sales or storage.
8. Communications and Compliance
Consignee agrees to receive text or email communications from Pure Aquaox related to reporting, delivery, and program updates.

9. Entire Agreement
This document represents the entire agreement between the parties and supersedes any prior oral or written understanding.
